home medical care for over 20 years. As a Primary Care Physician,

being of older adults. Dr. Bautista will work closely with the patient’s
medical team of specialists, nurses and therapists to ensure a seamless and comprehensive healthcare experience.

We Offer Flexible Coverage
We accept Traditional Medicare and supplemental
insurance policies. Medicare Advantage plans can ONLY be accepted if it is a PPO policy with out of network benefits.
No HMO policies are accepted at this time.
